Who is Eligible? Hospice is available for those persons with a terminal illness and a life expectancy of six months or less. We serve persons who are residents of Grant County. There should also be someone available who will be able to assist with day to day care. How are Hospice Services Paid For? Medicare Medicaid Private Insurance We also look to our community for support through fundraising, donatiosn and memorials.What Services are offered? In Your Home Regular visits by a Registered Nurse with 24 hour on-call service available Specialized care by Nurse Aides Volunteer Support Grief Counseling and Bereavement care for the patient and their family members Medical Equiopment and supplies Prescribed medications related to the terminal illness State of the art management of pain Respite care, occasional 2-3 hour periods of relief for the primary care giverin the Nursing Home The above described benefits are also available to nursing home residents with the exception of respite care. In the Hospital Hospice patients hospitalized at the advice of their physicians continue receiving hospice care and expertise in pain and symptom control. What is an Interdisciplinary Team Approach? The interdisciplinary approach utilizes a team of professionals including physicians, nurses, nurse aids, social workers, clergy, therapists, and specially t rained volunteers. Our team works to meet the need of patients and their loved ones. It is our desire to companion patients and their families as they encounter the stress and concerns arising from terminal illness. Blue Mountain Hospice hopes to be a refuge along this often difficult journey. We offer support designed to nurture your physical, emotional, and spiritual needs. |
